Bridgepoint Health

Bridges to Wellness! Located in central east Toronto, Bridgepoint Health (www.bridgepointhealth.ca) offers self management programs for individuals living with complex, chronic conditions. The next session will commence in late April 2007 and consists of six weekly sessions of approximately 2˝ hours in length. Participants are welcome to involve family members in the group as well. Group size will be approximately 8 to 16 participants.

Baycrest Centre for Geriatric Carebaycrest centre

Baycrest offers three self-management programs for people living in the community with chronic illnesses. In a group format, participants develop problem solving skills, learn to set meaningful goals for themselves and identify community resources and supports.

  • PEIP – Parkinson’s Early Intervention Program
  • MOST – Moving On after Stroke

Toronto Rehabilitation Institute offers Life with MS – a 10-week self-management program led by Toronto Rehab professionals. In a group format, you will develop the skills and attitudes to maximize your well-being and build confidence in making successful adaptations to MS. Centre for Movement Disorders

The Centre for Movement Disorders offers two Self-Management Programs for people with Parkinson’s Disease (PD) and their partners:

The Early Management Program is for people diagnosed in the last three years with mild symptoms. The Safe Mobility and Falls Prevention Program is for people with PD who are having difficulty with mobility and walking.

Both Self- Management Programs include group discussion and non- didactic education, goal setting and skill- building, and specialised exercise. They each run for eight weeks and are facilitated by a physiotherapist and a trained volunteer with PD.

Toronto Rehabilitation Institute

Trillium Health Centre offers Patient Self-Management sessions at community health centres in Mississauga, in cooperation with the "Optimizing Life with Long-Term Conditions" program and the Institute for Optimizing Health Outcomes.
