OPSMN News: January 2008

OPSMN announces: First Time in Ontario - Master Training Opportunities in Stanford’s Chronic Disease Self-Management Program from Ontario-based T-Trainers. Durhane Wong-Rieger and Mary Kim received T-Trainer status in December 2007 and they are now qualified to offer the 5-day Train-the-Trainer Program. Contact us at info@optimizinghealth.org.

The Canadian Cancer Society – Ontario Division, in partnership with Optimizing Health, is conducting a pilot program to assess the value of the CDSMP for individuals post-cancer-treatment. Pilot workshops were completed this past summer in Burlington, Sudbury, and Toronto. Click here for the Toronto promotional flyer.

From North York General Hospital: Journey into Well-being, the Chronic Disease Self Management Program is a six-week workshop for adults with chronic health problems such as heart disease, arthritis, diabetes, asthma, bronchitis, emphysema and other conditions. Three workshops will be offered to those who are interested: The 1st workshop will be at the Seniors' Health Centre starting Thursday, January 17, 2008 from 12:30-3:00 for six weeks. The 2nd workshop will be at the North York YMCA at Bayview and Sheppard starting Tuesday, January 29, 2008 from 1:00-3:30 for six weeks. The 3rd workship will be at the Bathurst Jewish Community Centre starting Wednesday, March 5, 2008 for six weeks (time to be determined). The Arthritis Society is registering for 2008 classes in the Arthritis Self-Management Program. The Arthritis Self-Management Program is a six week program held once a week for 2 hours.
The Arthritis Self-Management Program is designed to help you better understand your arthritis, learn ways to cope with chronic pain and take a more active role in managing your arthritis.
If you have one or more of the many types of arthritis, you will benefit from participating in the Arthritis Self-Management Program. Your partner, family member, friend or other support person is encouraged to attend the program with you. As a participant in the class. They too will benefit from learning about your arthritis and how it affects you.

Baycrest offers three Self Management Programs: one for people who have had a stroke been through rehabilitation , one for people with Arthritis and one for people with Parkinson's Disease. Living a Healthy Life with Chronic Conditions Self-Management Program via Telehealth video-conferencing is a free education program for people with chronic disease to learn the skills needed for the daily management of long-term illness. Each course will be 2.5 hours a week for 6 weeks and will be offered using video-conferencing from a telehealth studio in each participating community. Participants in this program will receive a free course textbook and will also be asked to complete a telephone survey four times over the next year about their experiences in the program, health status, health behaviours and use of health care services.
Individuals who are 55 years of age or older and have one or more of the following conditions may benefit from this program:

  • Arthritis
  • Stroke
  • Heart Disease (e.g. coronary artery disease, congestive heart failure)
  • Lung Disease (e.g. asthma, chronic bronchitis, COPD, emphysema)
Participants of this program will learn:
  • How to manage their symptoms
  • How to eat well and live more actively
  • How to communicate effectively with their health care team
  • How to manage fear, anger, frustration, and depression
  • How to make daily tasks easier
  • How to get more out of life
